  • Benefit Coverage is provided by Canada Life, and your plan is serviced by ICBA Benefits.
  • The renewal date for the plan is January 1 of each year.

Are all applicants covered by a provincial government health care plan?

Applicant

? The definition of earnings is the average annual income as an individual that would be lost if disabled. The income will be defined as those lost from the current employer due to a disability. Proof of loss of income is required at time of claim. Those that cannot prove loss of earnings, or to the amount insured may not be deemed eligible for the benefit.
Unfortunately, applicants age 65 and up or 17 and under are not eligible for this plan. Please contact us for other options

Life Insurance

All options cover:

  • $25,000 coverage
  • Reduced by 50% at age 65
  • Terminates at age 75
Accidental Death & Dismemberment

All options cover:

  • $25,000 coverage
  • Reduced by 50% at age 65
  • Terminates at age 75
Long Term Disability

All options cover:

  • 66.67% of monthly earnings up to $3,000
  • Elimination period: 120 days
  • Benefits paid for up to 5 years
  • Terminates age 65
Dependent Life

All options cover:

  • $10,000 coverage for spouse
  • $5,000 per child
  • Terminates at employee age 75

Extended Health Care (Choose one)

Option 1

  • $250 annual deductible
  • 70% coverage except travel insurance and Vision Care
  • 80% for drugs purchased within Canada Life's preferred pharmacy network
Option 2

  • $100 annual deductible
  • 80% coverage except travel insurance and vision care
  • 90% for drugs purchased within Canada Life's preferred pharmacy network
Option 3

  • No Deductible
  • 90% coverage except travel insurance and vision care
  • 100% for drugs purchased within Canada Life's preferred pharmacy network
ALL OPTIONS COVER ELIGIBLE DEPENDENTS:
  • Brand name drugs paid at generic equivalent
  • Up to $5000/year for drugs, including vaccines
  • $300/year per paramedical practitioner (e.g. chiropractor, physiotherapist, massage therapy)
  • Medical Aids & Equipment (e.g. $300 every 12 months for Foot Orthotics)
  • 100% for vision care up to: $200 every 24 months for adults and $200 every 12 months for dependent children
  • 100% Travel insurance
  • Terminates at age 75

Dental Care (Choose one)

Option 1

  • 80% Basic up to $1500/year
Option 2

  • 80% Basic and 50% Major up to $1500/year
  • 50% Orthodontia, up to $2000/lifetime
Option 3

  • 100% Basic and 50% Major up to $1500
  • 50% Orthodontia, up to $2000/lifetime
ALL OPTIONS COVER ELIGIBLE DEPENDENTS:
  • 2 recall visits
  • Up to 14 units of scaling/year
  • Terminates at age 75

Optional Coverage: Short Term Disability

? Short Term Disability (STD) provides income for a covered plan member should they be unable to work for a short period of time due to hospitalization, an accident, or becoming ill. STD provides crucial, short term income replacement to ease any financial strain and burden while a plan member is recovering.
Option 1
  • 66.67% of salary up to the current EI maximum
  • Benefit pays on 1st day of hospitalization, 8th day off illness/injury for up to 17 weeks
  • Terminates at age 75

Optional Life Insurance Coverage

While there is Life Insurance coverage included in the Entrepreneur plan, Optional Life Insurance increases the benefit amount paid to the named beneficiary in the event of an insured person’s death. Depending upon your life stage and status, extending your Life Insurance coverage can provide additional piece of mind to cover expenses and obligations for your loved ones and beneficiaries.

Optional AD&D Coverage

While there is Accidental Death and Dismemberment (AD&D) coverage included in your Entrepreneur plan, Optional AD&D coverage provides additional financial assistance for injury or bodily harm as the result of an accident to a plan member.
